SOCIAL WORKER (PT).
Description:
Part-time 20-30 hours per week. LSW will provide trauma-informed outpatient counseling to youth/adolescent in a residential group home setting and community clients. Responsibilities include conducting diagnostic assessments and treatment planning. Creates, reviews and updates care plans and progress notes. Actively participates in the weekly supervision process. Individual/group counseling, professional documentation, and continuous quality improvement activities. Works with the client, family and other team members to plan discharge. Conducts in-service programs to educate staff regarding psycho-social issues and client rights.
Ideal Candidate:
• Bachelor’s Degree in Social Work, Counseling or related field
• Ohio Licensure (independent and dependent considered)
• Experience with youth/adolescent patient population
• LSW license / certificate ***Supervision hours provided for therapists on dependent licensure!***
Hourly Rate: $14 - $16 (Based on Experience)
